This is the whole of it. There is no scoring model deciding whether a paraphrase is close enough, and no fallback that turns an unlocatable quote into a vaguer sentence that no longer needs one.

The claim binds to this version, never to “current”. Editing the file in SharePoint produces a new record rather than changing this one.
The three outcomes
Kept apart on purpose
Collapsing the third into the first is where confident nonsense comes from.
Located
The quote was found in the source text, character for character, after whitespace is normalised.
The span travels with it
Character offsets are recorded, so a reviewer opens the document and lands on the text.
So does the version
Offsets are meaningless without the version they refer to, so both are stored together.
It stays checkable later
Because the answer points at a version rather than at a file, it still resolves after the file changes.
Not located
The quote was looked for and was not in the source.
Flagged, not deleted
Removing it would hide that the system tried and failed, which is the thing a reviewer most needs to see.
Never softened
There is no path that rewrites an unsupported claim into something blander that escapes the check.
Counted on the run
The reviewer sees how many claims failed, not only the ones that passed.
Not checked
The source could not be read when the answer was produced.
Permissions move
A document readable on Monday can be walled on Tuesday, and the honest result is that nobody checked.
History expires
When a version has aged out of the provider's retention window, the system says so rather than reading a different version.
A limitation is a result
Typed limitations come back attached to the document, instead of disappearing into a confident answer.
What this costs you
Fewer answers, and that is the point
A system with this rule will decline to answer things a looser one would answer fluently. That is the trade, stated plainly rather than buried.
It will say it does not know
When the record does not contain the answer, nothing is produced. You get a gap rather than a plausible paragraph.
It cannot summarise what it cannot read
A scanned page with no usable text is a limitation the system reports, not a document it guesses at.
It does not argue
